What is gastro-oesophageal reflux?

Gastro-esophageal reflux is a condition where the lower esophageal sphincter (the muscular ring at the lower end of the esophagus) is abnormally relaxed and allows the stomach's acidic contents to flow back or 'reflux' into the gullet (esophagus). Oesophagitis

Oesophagitis is inflammation of the inner lining of the esophagus caused by repeated episodes of gastro-esophageal acid reflux.

The symptoms of gastro-esophageal reflux oesophagitis?

Heartburn is the most noticeable symptom among the number of symptoms of gastro-esophageal reflux disease (often just called reflux). It is estimated that about one third of adults experience heartburn at least once a month, with about 10 per cent of the population having heartburn once a week and 5 per cent at least daily.

Heartburn is a burning pain that is generally felt in the chest area behind the breastbone. The burning pain moves upwards during an episode of reflux. It can sometimes be felt in the throat. The pain of heartburn is usually provoked by eating or by bending over or straining.

Other symptoms of reflux include the following:-

  • Regurgitation of food or fluid into the mouth. This often leaves a bitter taste in the mouth. One may not be aware of bringing up anything but a bitter taste which appears in the mouth.
  • Water brash is the sudden appearance of an excess of salty or tasteless fluid in the mouth. This fluid is caused by an excessive production of saliva.
  • Belching. Many people with reflux suffer from excessive belching.
  • Chest pain that is similar to a heart attack is another symptom of reflux that is generally caused by acid reflux from the stomach into the esophagus. Always remember that your doctor should check any chest pain, even if it is associated with a gastrointestinal disorder rather than a heart condition.
  • Hoarseness or change in voice can occur when stomach acid reaches the mouth and throat causing laryngitis. You may wake up with a hoarse voice because you had reflux during the night and find that your voice gets better during the day.
  • Sore throat or dry cough. This is caused by the acidic stomach contents irritating the throat.
  • Night-time coughing or choking can happen when the stomach contents enter the windpipe and the lungs while you are sleeping. You may wake up suddenly feeling that you are choking.
  • Asthma attacks. In some people with asthma, the symptoms of wheezing and shortness of breath are made worse by reflux. Often treating the reflux condition will greatly improve the asthmatic symptoms.
  • Painful swallowing. In some cases of reflux, there may be difficulty in swallowing (dysphasia) or pain when swallowing (odynophagia). The pain is felt as the mouthful of food is swallowed and travels down the esophagus. The pain may be caused by ulceration of the esophagus brought about by it being exposed to acid over time. The pain may also be accompanied by a feeling that the food has got stuck in the esophagus. This feeling of a blockage is sometimes due to narrowing (a stricture) of the esophagus.
  • Vomiting of blood. This is not a common symptom of reflux and usually happens only in severe cases. Bleeding in the esophagus can cause vomiting of blood that is bright red or has the appearance of coffee grounds.

 

When does gastro-oesophageal reflux occur?

Symptoms typically occur after over eating a large or fatty meal or drinking alcohol.
Lying down, bending over or bending and lifting can all cause reflux.
It is made worse by smoking.

How does Gastro Acid Reflux's stomach acid travel up the oesophagus?

When the oesophagus isn’t being used, the sphincters contract so that the contents of your stomach don’t flow back up to your mouth. When you swallow, these sphincters expand or relax to let food pass into your stomach.

When food reaches the stomach, it is partially digested by stomach acid and enzymes and then passed into the small intestine for further digestion. People who suffer from reflux have the acidic stomach contents regurgitate back up into the esophagus. The stomach acid can cause inflammation and damage to the esophagus. This is because the esophagus does not have such a protective lining as the stomach.
 
If the reflux oesophagitis is allowed to continue there can be serious consequences as the esophagus sustains more and more damage. Regular exposure to the acidic stomach contents may cause the cells in the esophagus to change. A gastroenterologist can see these changes when they carry out an endoscope (where a long tube is passed down your throat to view the esophagus and stomach).

The esophagus may become ulcerated or scarred. Build up of scar tissue can lead to the esophagus being narrowed. This narrowing is called a stricture and can lead to difficulty in swallowing.
 
Regular exposure of the esophagus to acid from the stomach can change the lining of the esophagus so that it is more like the lining of the stomach and so more resistant to acid. This is called Barrett’s oesophagus, after the doctor who discovered it. However, these changes in the cells can increase the likelihood of cancer of the esophagus.

Which conditions can cause gastro-oesophageal reflux?

In many cases, the Gastro Acid Reflux disorder cannot be attributed to any specific cause but the following conditions can contribute to the problem.

  • Hiatus hernia- Gastro Acid Reflux-

The stomach pokes through the diaphragm, preventing the muscle fibres of the diaphragm from closing the lower end of the esophagus. The esophagus remains wide open which allows stomach acid to get into the esophagus.

 

  • Overweight- Gastro Acid Reflux-

If the person is overweight, the excessive fat in the abdominal cavity increases the pressure inside it. This causes the contents of the stomach to travel up into the gullet. Loss of weight reduces stomach acid reflux.

  • Pregnancy -Gastro Acid Reflux-

Because the uterus increases in size during pregnancy, it presses on the stomach, creating higher pressure inside it, which increases the tendency to reflux. In addition, hormonal changes lead to relaxation of the esophageal sphincter during pregnancy.

  • Big Meals - Gastro Acid Reflux-

The more the stomach is stretched by food, the higher the tendency to reflux. The tendency is also increased by eating fatty meals as fat delays gastric emptying. Try to avoid large rich meals, particularly in the evening and this will reduce the tendency to reflux.

  • Unsuitable Foods- Gastro Acid Reflux-

Chocolate, peppermint, coffee, fruit juices, sour, hot spicy food , solid nuts and alcohol and over eating prevent the esophageal sphincter from working properly.

  • Tobacco -Gastro Acid Reflux-

Tobacco prevents the oesophageal sphincter from working in order, reduces the rate at which the stomach empties and increases stomach acid production.

  • Constipation -Gastro Acid Reflux-

Constipation increases the tendency to reflux by raising pressure inside the stomach cavity.

  • Lying down-Gastro Acid Reflux Conditions-

The tendency to reflux increases when you are lying down. This is just due to gravity. A simple way to change that is to use a pillow under the mattress or to raise the head of your bed by 3 inches with blocks or a house brick under the bed frame.

 

Gastro-oesophageal reflux disorders diagnosed?

The symptoms of gastro-oesophageal reflux may be so obvious that no tests are needed. There are others scary test as well but we don't advise to do. If the doctor is in doubt, or if the symptoms are very troublesome, a gastroscopy may be considered. During the procedure oesophagitis, hiatus hernias, peptic ulcers and other conditions can be either found or ruled out.

Another possibility is to measure the acidity in the lower end of the esophagus during a 24-hour period. This will give an indication of how often and how long the reflux episodes last.

Gastro-oesophageal reflux can resemble those of a peptic ulcer, chest pains (angina pectoris), muscle pains, back problems, constipation, irritable bowel syndrome, gallstones, pancreatic disease etc. These conditions will sometimes have to be ruled out before the diagnosis can be made.

 

How to prevent gastro-oesophageal reflux?
  • Dietary Changes

    People with heartburn should first try lifestyle and dietary changes. In one study, 44% of patients who experienced symptoms of gastroesophageal reflux disease (GERD) reported improvement after changing their diet. Some suggestions are the following: People with heartburn should avoid or reduce consumption of foods and beverages that contain caffeine, chocolate, peppermint, spearmint, and alcohol. Both caffeine and decaffeinated coffee increase acid secretion.

  • All carbonated drinks increase the risk for GERD.

Although physicians often advise patients with GERD to cut down on fatty foods, studies are finding no evidence that a low-fat or high-fat meal make any difference in symptom exacerbation. Better studies are needed to confirm this. In any case, as a rule, it is always wise to avoid saturated fats (which are from animal products), and cut down on all fats if one is overweight.

  • Increasing protein may help strengthen muscles in the muscle valve. Patients should choose low-fat or skim dairy products, poultry, or fish, in such cases.
  • Whole grain products rich in selenium may have some protective role against dangerous cells changes in Barrett's esophagus.
  • Patients should have a diet rich in fruits and vegetables, although avoid acidic vegetables and fruits (e.g., oranges, lemons, grapefruit, pineapple, tomatoes).
  • Patients who have trouble swallowing should avoid tough meats, vegetables with skins, doughy bread, and pasta
  • Quitting smoking is essential.
  • People who are overweight should try to reduce food intake.
  • People with GERD should avoid tight clothing, particularly around the abdomen.
  • If possible, GERD patients should avoid nonsteroidal anti-inflammatory drugs (NSAIDs), such as aspirin, ibuprofen (Motrin, Advil), or naproxen (Aleve), among others. Tylenol (acetaminophen) is a good alternative pain reliever.
  • Although gum chewing is commonly believed to increase the risk for GERD symptoms, one study reported it might be helpful. Because saliva helps neutralize acid and contains a number of other factors that protect the esophagus, chewing gum 30 minutes after a meal has been found to help relieve heartburn and even protect against damage caused by GERD. Chewing on anything at all can help since it stimulates saliva production
